Why Local Reporting in 2026

During 2026, all you need is to — locally — report, document, analyze, and share learnings about incidents or near misses, until a suitable platform complying with 2027 requirements is in place. The four steps below walk you through exactly how to do that using tools you already have.

Follow These 4 Steps

AIMS Requirement: Date & Time Stamp

The email you receive in Step 3 will carry an automatic date and time stamp at the top — satisfying one of the 13 requirements by AIMS. Do not delete or alter the email header before saving.

Setup

Create Your Local Folder

Right-click on your Desktop and select New Folder. Name the new folder exactly:

This folder will serve as your secure local archive. Keep it on your work computer and back it up to an encrypted USB or secure network drive if possible.

Report

Access the Form & Fill Out the Report

Click the "Access Reporting Form" button below to open the incident report form. Complete all required fields describing the incident or near-miss event.

✉️  You will need to enter your email address — this is how your completed report will be delivered to you.

Submit

Submit — Then Check Your Email

When you finish filling out the form, click Submit. You will receive an email containing your completed report.

🕐  Important: The email will display the date and time stamp at the top — this is one of the 13 AIMS requirements. Do not forward or alter the email before saving.

Save as PDF

Print the Email to PDF & Save to Your Folder

Open the email you received. Then follow these steps to save it as a PDF:

If using Gmail:
Click the menu or the print icon in the top-right corner of the email In the print dialog, change Destination to Save as PDF Browse to and select the folder Incident Reports 2026 Click Save
If using Outlook or another email client:
Use File Print, then select Microsoft Print to PDF (Windows) or Save as PDF (Mac) as the printer, and save to your Incident Reports 2026 folder.

Your report is now permanently saved as a timestamped PDF and is ready to present during a pharmacy assessment if required.
